MDS Coordinator (Full-Time)

We are currently looking for a MDS Coordinator who will oversee scheduling of MDS assessments to assure compliance with federal and state requirements, review and certify completion of MDS assessments, coordinate all functions of the MDS process and participate in and/or conduct interdisciplinary meetings.  Coordinator will participate in pre – admission process, quality assessment and assurance program, and will manage the daily operations within the MDS department.  This will include and not be limited to human resources functions, budgeting process, purchasing, and coordinating with other facilities or corporate office.  Coordinator works under the direct supervision of the facility administrator.

Floor Technician (Full-Time)

We are currently looking for a Floor Tech who will execute the cleaning of facility floors to ensure an attractive, clean, and sanitary environment in accordance with laws, regulations and facility guidelines.

Responsibilities

  • Ensures that the floors and rooms within the facility are cleaned in the proper manner.
  • Follows all established safety procedures and precautions when performing housekeeping services.
  • Satisfactorily sweep, mop and buff floors as assigned.
  • Strip and wax floors according to schedule.

Restorative Certified Nursing Technician (Full-Time)

Responsibilities:

  • Coordinates restorative nursing care to residents.
  • Participates in the development of an individualized plan of care for assigned residents and ensures plan of care is followed by Restorative Aides.
  • Ensures documentation is completed for each treatment provided, refusal of treatment, reason(s) treatment could not be provided and progress notes as required.
  • Be familiar with and be able to train Restorative Aides on programs such as range of motion, ambulation, active and passive exercise, use of assisted devises, ADL programs, and feeding programs.
  • Involvement in family or resident teaching
  • Supervises Restorative Aides.
  • Responsible for maintaining fall records.
  • Responsible for completing all MDS sections related to restorative care.

Qualifications:

  • Must be a Licensed Practical Nurse or Registered Nursing in the state of TN

Dietary Aide (Full-Time)

We are currently looking for Dietary Aides who will assist in the preparation of food in quantities according to the menu, recipe and number of persons to be served, including setting up dining room as instructed and prepares items such as salads and beverages. Takes resident orders and serves meals to residents in a timely manner. Cleans resident eating areas, work areas, equipment and utensils according to the proper procedures, including collecting and stacking dirty dishes on cart and returns cart to kitchen, washing cooking equipment, carts, pots, pans, dishes and utensils, cleaning eating and prep surfaces, sweeping and moping floor, removing garbage, storing food in designated areas. Consistently work cooperatively with residents, resident’s representatives, facility staff, physicians, consultants and ancillary service providers.

Director of Nursing (Full-Time)

Job description

This position coordinates Nursing Services in accordance with Tennessee State Veterans’ Home Board polices and applicable state and federal laws. Supervisory responsibilities include interviewing, hiring and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

JOB DETAILS

  • Set resident care standards in accordance with accepted current standards of care to provide high quality care to residents.
  • Assess, direct and supervise residents’ care needs and assist in the development of individualized plans of resident care to include making recommendations for special rehabilitation programs, family involvement and the physician’s plan of care for the resident.
  • Responsible for development and implementation of nursing rehabilitation and restorative programs.
  • Assess resident responses to medication and treatments and make appropriate recommendations for nursing action to be implemented.
  • Conduct or participate in resident care conferences.
  • Direct, evaluate and supervise all resident care and initiate corrective action as necessary.
  • Conduct daily resident rounds and initiate corrective action and report problems to the Administrator as necessary.
  • Analyze Quality Indicator reports, identify concerns and implement corrective action to improve resident care.
  • Identify safety hazards and initiate corrective action. Responsible for analysis of resident incident and accident investigation reports to determine cause(s) and implement corrective action(s), when appropriate.
  • Responsible for the development of nursing service objectives and performance standards of nursing practice for each category of nursing personnel.
  • Direct and implement nursing service educational programs including, but not limited to, orientation and in-service for licensed and unlicensed nursing personnel.
  • Conduct staff meetings with all shifts for purposes of planning, coordinating and implementing nursing service policies and procedures.
  • Demonstrate consistent management of nursing service problems, emergency situations, and initiate “life-saving” measures in the absence of a physician.

  • The DON is responsible for the overall clinical services provided 24 hours a day, seven days per week.

RN Admissions Case Manager (Full-Time)

Job description

The RN Case Manager will facilitate the admission and marketing process. This individual will function as a liaison between the referring hospital/ health care institution to ensure appropriateness of the placement and continuity of care. Additionally, The RN Case Manager will develop an initial/ interim plan of care that addresses the Resident’s current and immediate needs.

Medical Records (Full-Time)

Job description

Department: Nursing

Type: Full-time

Reports To: Director of Nursing

Responsibilities:

  • Assist in organizing, planning and directing the medical records department in accordance with established policies and procedures.
  • Retrieve resident records (manually/electronically). Deliver as necessary.
  • Files information such as nurses’ notes, resident assessments, progress notes, laboratory reports, x-ray results, correspondence, etc., into resident charts.
  • Ensure incomplete records/charts are returned to appropriate departments or personnel for correction.
  • Assist in developing procedures to ensure resident records are properly completed, assembled, coded, signed, indexed, etc., before filing.
  • Establish a procedure to ensure resident charts/records do not leave the medical records room except as authorized in our policies and procedures.
  • Maintain a record of authorized information released from charts/records, i.e., type information, name of recipient, date, department, etc.
  • Abstract information from records as authorized/required for insurance companies, Medicare, VA, etc. in accordance with current Privacy Rules.
  • Answer telephone inquiries concerning medical records function.
  • Prepare written correspondence as necessary.
  • Retrieve medical records when requested by authorized personnel (i.e., physicians, nurses, government agencies and personnel, etc.)
  • Assure that medical records taken from the department are signed out and signed in upon return to the department.
  • File active and inactive records in accordance with established policies.
  • Perform secretarial duties for the committees of the facility as directed.
  • Maintain the confidentiality of all resident care information including protected health information.
  • Serves as HIPPA Officer for facility
  • Assist MDS Coordinator in collecting M.D. signatures for percent for Medicare upon admission and readmission
  • Works closely with billing-therapy-to assume ICD-9- coding is complete
  • Provides resident information to the business office regarding Medicare part A and B billing and 70% veterans.

  • Coordinates restorative nursing care to residents.

The Tennessee State Veterans’ Homes Board (TSVHB) has an immediate opening for an Administrator at our long-term nursing home in Cleveland, TN. The Administrator is responsible for the overall operation of the facility and coordinates objective-focused teamwork in all departments. The Administrator must possess skills, knowledge and abilities to account for the individual departments and be able to provide leadership, direction and motivation to a qualified staff operating with autonomy in their daily functions. This position reports to the Executive Director and will also work directly with the management team.

We are looking for a responsible Administrator to establish and maintain systems that are effective and efficient to operate a skilled nursing facility (SNF) in a manner to safely meet resident's needs in compliance with federal and state law, and local requirements. The Administrator must be able to establish and maintain systems which are effective and efficient to operate the facility in a financially sound manner.

Our ideal candidate has significant experience as the Administrator of a facility of more than 100 beds. Applicants must possess a valid Tennessee nursing home administrator license.
