The RN Case Manager will facilitate the admission and marketing process. This individual will function as a liaison between the referring hospital/health care institution to ensure appropriateness of the placement and continuity of care. Additionally, The RN Case Manager will develop an initial/interim plan of care that addresses the Resident’s current
and immediate needs.
QUALIFICATIONSUtilization Management/Discharge Planning/Clinical Case management experience preferred. Admission and marketing experience preferred.

EDUCATIONAL REQUIREMENTSAssociate’s or Bachelor’s degree in nursing.
CERTIFICATIONS AND LICENSESTennessee RN license in good standing.
