This position will serve as a liaison between the facility, local discharge planners, families, and health care providers.
Case manager will review all referrals and make determinations of appropriate admissions.
Complete PAE’s and PASRRs on all admission/in house residents as needed.
Complete on site visits as needed.
Assist in completing admissions and develop interim care plans.
Participate in on call schedule
Assist in other duties as needed

Minimum of (5) five years current experience in post-acute and/or Managed Care experience preferred
Utilization Management/Discharge Planning/Clinical Case Management experience preferred.
